Chaperone Information

 

Chaperone Information & Guidelines Booklet

TJ Band Patrons is required to follow the West Jefferson Hills School District Clearance Policy (Policy No. 124) for volunteers.  If you would like to volunteer to chaperone the band during any band events, the appropriate clearance paperwork must be on file. Filing the appropriate paperwork will allow you to be added to the volunteer chaperone list and the Chaperone Coordinator will contact you as needed. 

 

Required Clearances

Note:  If you currently have clearances and wish to know if they are sufficient to volunteer, you must contact the school district’s Director of Human Resources (see below).

 

Act 34 - State Criminal Background Check

· Cost is $10.00

· Visit the Pennsylvania State Police website

· Two options:  Online with a credit card or via downloadable form with a check or money order mailed to PSP

· Online is normally the fastest option, with an immediate response 85% of the time (per PSP website)

 

Act 151 - State Child Abuse History Clearance

· Cost is $10.00 paid by money order or business check only (no personal checks)

· Visit the Pennsylvania Department of Education website for information

· Visit the Pennsylvania Department of Public Welfare website for the form and instructions

-       Link to download the form is after the instructions

-       A copy of the State Criminal Background Check must be attached (issued within one year of the Child Abuse History Clearance request)

-       Do not obtain FBI clearance

 

Once You Receive Both Clearances

· Retain the originals for your records

· Provide copies of both clearances to the Administration Building, TJHS Principal’s Office, and TJ Band Patrons

· On the TJ Band Patrons copies, black out your Social Security Number
